In response, an immune response is triggered that damages the small intestine and prevents normal absorption of nutrients. Symptoms can be severe or almost imperceptible, so accurate diagnosis is key.<\/p>\n<p data-start=\"869\" data-end=\"1194\">The most important rule of modern medicine: <strong data-start=\"909\" data-end=\"987\">Celiac disease testing is only done when a person consumes gluten.<\/strong> If gluten is eliminated from the diet in advance, the tests may become unreliable and give a false-negative result. That is why the diagnosis includes several consecutive stages, each of which complements the previous one.<\/p>\n<h2 data-start=\"1196\" data-end=\"1238\">When to get tested for celiac disease<\/h2>\n<p data-start=\"1240\" data-end=\"1278\">It is worth paying attention if a person has:<\/p>\n<p data-start=\"1280\" data-end=\"1619\">\u2022 abdominal pain, bloating, gas<br data-start=\"1318\" data-end=\"1321\" \/>\u2022 chronic diarrhea or loose stools<br data-start=\"1357\" data-end=\"1360\" \/>\u2022 <a href=\"https:\/\/medilab.km.ua\/en\/iron-deficiency-causes-symptoms-and-treatment-methods\/\">iron deficiency<\/a> or <a href=\"https:\/\/medilab.km.ua\/en\/b12-deficiency-anemia-signs-causes-diagnosis-and-treatment\/\">anemia<\/a> of unknown origin<br data-start=\"1409\" data-end=\"1412\" \/>\u2022 impaired absorption of nutrients<br data-start=\"1453\" data-end=\"1456\" \/>\u2022 fatigue, irritability, mood swings<br data-start=\"1496\" data-end=\"1499\" \/>\u2022 weight loss<br data-start=\"1512\" data-end=\"1515\" \/>\u2022 frequent stomatitis<br data-start=\"1532\" data-end=\"1535\" \/>\u2022 skin rash similar to dermatitis herpetiformis<br data-start=\"1590\" data-end=\"1593\" \/>\u2022 growth retardation in children<\/p>\n<p data-start=\"1621\" data-end=\"1875\">Celiac disease can manifest with extraintestinal symptoms, including osteopenia, decreased immunity, and elevated liver enzymes. That is why international organizations include it in the differential diagnosis even when digestive problems are absent.<\/p>\n<h2 data-start=\"1877\" data-end=\"1937\">Blood tests: antibodies that help detect celiac disease<\/h2>\n<p data-start=\"1939\" data-end=\"2044\">The first and most important stage is serological tests, i.e. the detection of antibodies in the blood. Main indicators:<\/p>\n<p data-start=\"2046\" data-end=\"2193\">\u2022 tTG-IgA \u2014 antibodies to tissue transglutaminase<br data-start=\"2096\" data-end=\"2099\" \/>\u2022 EMA-IgA \u2014 endomysial antibodies<br data-start=\"2133\" data-end=\"2136\" \/>\u2022 DGP-IgG \u2014 antibodies to deaminated gliadin peptides<\/p>\n<p data-start=\"2195\" data-end=\"2454\">tTG-IgA is the most sensitive and specific test recommended as the first step in diagnosis. EMA-IgA is performed in difficult or doubtful cases, as it has almost 100% specificity. DGP-IgG is used in children under 3 years of age and in patients with IgA deficiency.<\/p>\n<p data-start=\"2456\" data-end=\"2652\">A total IgA test is mandatory. If a genetic deficiency of immunoglobulin A is detected, then tTG-IgA and EMA-IgA values may be false - in this case, IgG tests are used.<\/p>\n<h2 data-start=\"2654\" data-end=\"2691\">HLA-DQ2 and HLA-DQ8 genetic tests<\/h2>\n<p data-start=\"2693\" data-end=\"2765\">Genetic testing does not diagnose, but helps assess risks.<\/p>\n<p data-start=\"2767\" data-end=\"2845\">\u2022 95% of people with celiac disease have the HLA-DQ2 gene<br data-start=\"2809\" data-end=\"2812\" \/>\u2022 most others \u2014 HLA-DQ8 gene<\/p>\n<p data-start=\"2847\" data-end=\"2899\">If both genes are missing, celiac disease is almost impossible.<\/p>\n<p data-start=\"2901\" data-end=\"2940\">Genetic testing is prescribed when:<\/p>\n<p data-start=\"2942\" data-end=\"3079\">\u2022 the person has already switched to a gluten-free diet<br data-start=\"2985\" data-end=\"2988\" \/>\u2022 blood test results are inconsistent<br data-start=\"3027\" data-end=\"3030\" \/>\u2022 risk assessment of first-degree relatives is required<\/p>\n<h2 data-start=\"3081\" data-end=\"3142\">Small bowel biopsy: a key step in confirming the diagnosis<\/h2>\n<p data-start=\"3144\" data-end=\"3265\">A biopsy is performed during gastroscopy and allows you to assess the condition of the mucous membrane of the small intestine. Under the microscope, they look for:<\/p>\n<p data-start=\"3267\" data-end=\"3359\">\u2022 villous atrophy<br data-start=\"3285\" data-end=\"3288\" \/>\u2022 excessive number of lymphocytes<br data-start=\"3319\" data-end=\"3322\" \/>\u2022 changes in the structure of the mucous membrane<\/p>\n<p data-start=\"3361\" data-end=\"3561\">This is the \u00abgold standard\u00bb for diagnosing celiac disease in adults. In difficult cases, your doctor may recommend a gluten challenge \u2014 a controlled return of gluten to the diet for 6\u20138 weeks.<\/p>\n<h2 data-start=\"3993\" data-end=\"4043\">Dermatitis herpetiformis: a cutaneous manifestation of celiac disease<\/h2>\n<p data-start=\"4045\" data-end=\"4309\">This is a chronic, itchy rash that often appears on the elbows, knees, buttocks, or back. A skin biopsy with immunostaining is done to confirm the diagnosis. This variant of celiac disease responds to a gluten-free diet in the same way as the classic intestinal variant.<\/p>\n<h2 data-start=\"4311\" data-end=\"4341\">What may resemble celiac
