Having felt a lump on the neck, under the jaw, behind the ear, in the armpit or groin area, a person involuntarily thinks about the worst. However, such an increase is not always associated with dangerous diseases. Most often it is an immune response to infection, inflammation, dental problems, throat diseases, skin lesions or a recent viral illness.

Lymph nodes are an important part of the immune system—they filter lymph and help the body fight viruses, bacteria, and other irritants. According to the Mayo Clinic, the most common cause of enlarged lymph nodes is infection, and less commonly, immune disorders or cancer.

It is worth remembering: you cannot diagnose yourself based only on size or pain. A painful node after a cold and a painless enlargement of several groups of lymph nodes over several months are completely different clinical cases. Therefore, the doctor always evaluates not only the node itself, but also the accompanying symptoms, duration, localization, blood test results and, if necessary, instrumental examination data.

When an enlarged lymph node may be a reaction to an infection

Lymph nodes most often enlarge near an area where there is inflammation or infection. For example, nodes in the neck may respond to a sore throat, pharyngitis, dental infection, ear infection, or a viral cold. Axillary lymph nodes sometimes enlarge when the skin of the arm is inflamed or after irritation in the armpit area. Inguinal nodes may respond to skin infections of the legs, genital infections, or inflammatory processes in the lower body.

There is less cause for alarm if the lymph node:

appeared during a cold, sore throat or runny nose;

painful when pressed;

movable under the skin;

gradually decreases after recovery;

not accompanied by sudden weight loss, night sweats, or prolonged fever;

has a clear connection with local inflammation.

But even after an infection, a lymph node doesn't always go away right away. Sometimes it can remain enlarged for a while, gradually shrinking. However, if a node grows, becomes firm, doesn't move, or persists for a long time without an obvious cause, it's best to see a doctor.

When to see a doctor

The Mayo Clinic advises seeking medical evaluation if lymph nodes appear for no apparent reason, continue to enlarge or remain enlarged for two to four weeks, are hard, fixed, or are accompanied by a persistent fever, night sweats, or unexplained weight loss. Seek immediate medical attention if there is difficulty breathing or swallowing.

The examination should not be postponed if:

the lymph node enlarges without a cold or other obvious cause;

the knot lasts for more than a few weeks;

the knot is hard or almost immobile;

several groups of lymph nodes are enlarged;

there is night sweats;

there is a temperature for no apparent reason;

there is weight loss;

there is severe weakness;

there are frequent infections;

there is a rash;

there is a sore throat that does not go away for a long time;

there is bleeding, bruising, or paleness;

the node is located above the collarbone;

difficulty swallowing or breathing appeared.

The presence of such symptoms does not automatically mean a serious illness. But it is a reason not to limit yourself to observation at home.

What the doctor assesses during the examination

First of all, the doctor is not in a hurry to prescribe tests - first he conducts an examination and asks the patient questions. Everything is important: where the node is located, what it feels like to the touch, whether it hurts, whether it moves, whether there are other nodes nearby, whether there are any signs of infection.

Typical questions from the doctor:

when the node appeared;

were there any symptoms of SARS (sore throat, runny nose, cough, fever);

are there any problems with your teeth;

whether there were any skin injuries, scratches, animal or tick bites;

Have you traveled recently?;

Are you concerned about night sweats or weight loss?;

What medications do you take;

are there any chronic diseases;

Has anything like this happened before?.

According to MedlinePlus, not only the node itself (size, mobility, tenderness, density) is evaluated, but also the general condition. If necessary, a complete blood count with a formula, as well as liver and kidney tests are prescribed.

What blood tests may be needed?

Lab tests can help determine if there are signs of infection, inflammation, anemia, changes in blood cells, or problems with internal organs. The Mayo Clinic notes that a complete blood count and other tests can help find the cause of enlarged lymph nodes.

Most often, a doctor can start with basic tests.

Examination

What helps to evaluate

Complete blood count

White blood cells, hemoglobin, platelets, signs of infection, anemia, or blood changes

Leukocyte formula

Ratio of different types of white blood cells

C-reactive protein

Activity of the inflammatory process

Erythrocyte sedimentation rate

Nonspecific sign of inflammation or chronic process

Biochemical blood test

General condition of the body, protein metabolism, liver and kidney function

Liver tests

May be needed if viral infections or systemic conditions are suspected

Tests for viral infections

May be needed for prolonged fever, weakness, rash, or enlargement of multiple groups of nodes

Immunological indicators

May be appropriate if an autoimmune process is suspected

Additional hematology tests

Prescribed for changes in general blood tests or suspected blood diseases

One of the first steps is often complete blood count, because it helps evaluate blood cells and see if there are signs of infection, anemia, or other abnormalities.

When may tests for infections be needed?

Enlarged lymph nodes are often associated with infections. These can include acute respiratory viruses, infectious mononucleosis, toxoplasmosis, tuberculosis, HIV, viral hepatitis, skin infections, sexually transmitted infections, or other pathogens, depending on the symptoms.

The doctor may order infection tests if there is:

prolonged fever;

sore throat and significant weakness;

enlargement of several groups of lymph nodes;

rash;

night sweats;

emaciation;

contact with sick people;

risk of blood-borne or sexually transmitted infections;

enlarged liver or spleen;

changes in liver parameters;

prolonged course of symptoms.

In some cases, the doctor may recommend HIV blood test, especially if the lymph node enlargement is long-lasting, widespread, or there are risk factors. This does not mean that HIV is the most likely cause, but such a test can be an important part of ruling out infections.

When is an ultrasound of the lymph nodes needed?

Ultrasound can help assess the size, shape, structure of a lymph node, its boundaries, blood supply, and features that cannot be reliably determined by palpation alone. This examination is often ordered if the node does not shrink for a long time, has an unclear cause, is growing, or raises doubts during examination.

Ultrasound may be appropriate if:

the lymph node persists for more than a few weeks;

the node is increasing;

the knot is tight;

there are several enlarged nodes;

atypical localization;

it is necessary to distinguish a lymph node from a cyst, fatty lump or other formation;

the doctor plans further follow-up over time;

It is necessary to determine whether there are grounds for a biopsy.

Ultrasound does not always establish a definitive diagnosis, but it helps the doctor decide whether additional methods can be observed or needed.

When a biopsy may be needed

A biopsy is a test of lymph node tissue. It is not recommended for all patients with enlarged lymph nodes. However, it may be necessary if the cause remains unclear, the node has suspicious signs, does not shrink for a long time, or there are symptoms that require the exclusion of serious diseases.

The Mayo Clinic notes that if necessary, your doctor may recommend a lymph node biopsy for microscopic examination. Imaging tests, such as an X-ray or CT scan, may also be used to look for possible sources of infection or tumor.

A biopsy may be considered if:

the node does not shrink after observation or treatment;

the knot is solid and fixed;

there is an enlargement of the supraclavicular lymph node;

there is night sweats, fever, weight loss;

there are significant changes in blood tests;

there is a suspicion of lymphoma or other cancer;

Ultrasound or CT scan results require clarification.

It is important not to demand a biopsy immediately without indications and not to postpone it if the doctor sees real reasons. This is a diagnostic method used when less invasive examinations do not provide a sufficient answer.

When enlarged lymph nodes may be associated with blood diseases

In most cases, enlarged lymph nodes have an infectious or inflammatory cause. However, sometimes they can be a manifestation of diseases of the blood or lymphatic system. That is why the doctor pays attention not only to the node itself, but also to the general condition of the person.

They can alert:

painless enlargement of lymph nodes;

increasing several groups of nodes;

prolonged fever;

night sweats;

unexplained weight loss;

severe fatigue;

itching of the skin;

frequent infections;

pallor;

bruising or bleeding;

changes in complete blood count.

The National Cancer Institute notes that enlarged lymph nodes may be a symptom that requires medical evaluation, although such symptoms are often caused by other conditions, not cancer.

If there are such symptoms, the doctor may prescribe advanced blood tests, ultrasound, CT scan, or refer you to a hematologist.

What not to do yourself

When a person finds an enlarged lymph node, their first reaction is often to constantly check it. But frequent pressure can irritate the tissue and keep it feeling sore or enlarged.

You should not do it yourself:

constantly press on the lymph node;

warm the enlarged node;

taking antibiotics without a prescription;

starting antiviral drugs without a diagnosis;

apply aggressive ointments;

postpone the examination if the node grows;

interpret tests without a doctor;

conclude about cancer only by the presence of a lymph node.

If a lump is painful due to a cold, it is not always dangerous. If it is painless but persists for a long time or is accompanied by general symptoms, this also does not automatically mean oncology, but it needs to be checked.

When follow-up is needed

Sometimes, after examination and tests, the doctor does not prescribe complex methods immediately, but recommends observation over time. This may be appropriate if the node appeared after an infection, there are no alarming symptoms, the tests are calm, and the lymph node itself is gradually decreasing.

A repeat examination may be necessary if:

the lymph node does not shrink;

the node is increasing;

new nodes appeared;

a fever has developed;

night sweats appeared;

weight has changed;

the general blood test has worsened;

new symptoms appeared.

Dynamics is very important. One review shows the current state, and a reassessment helps to understand whether the process is passing or requires deeper diagnostics.

What basic examination route might be logical?

There is no single list of tests for everyone with enlarged lymph nodes. But often the diagnosis moves gradually.

Typically, the doctor may do the following:

examination and history taking;

assessment of the location and size of the lymph node;

complete blood count with leukocyte formula;

C-reactive protein or other markers of inflammation;

biochemical indicators as needed;

tests for infections based on symptoms;

Ultrasound of lymph nodes;

consultation with a specialist;

biopsy or CT scan, if indicated.

This approach helps not to miss serious causes and at the same time avoid prescribing unnecessary tests where the lymph node is a normal reaction to an infection.

Enlarged lymph nodes can be a completely natural immune response to infection, but sometimes it signals a condition that requires more thorough laboratory and hardware diagnostics. If the node is painful and appeared against the background of a cold, observation according to the doctor's appointment is usually enough. However, if it does not go away for a long time, continues to grow, has become dense, has arisen for no apparent reason, or is accompanied by fever, night sweats, general weakness, or unexplained weight loss, then a timely examination helps turn anxiety into a clear medical plan of action.

An enlarged lymph node is often a cause for concern, especially when it occurs unexpectedly or does not decrease for a long time.
