Their determination is of greatest practical importance when examining primary biliary cholangitis (PBC) — a chronic autoimmune liver disease associated with damage to small intrahepatic bile ducts.

AMA is found in most people with PBC, so their determination is an important part of the laboratory diagnosis of this disease. The analysis can be especially informative when there is a simultaneous increase in alkaline phosphatase (ALP) or other indicators that may indicate impaired bile outflow.

What does the AMA-M2 analysis show?

The test detects the presence of antimitochondrial antibodies of the M2 type in the blood. Their detection may indicate an autoimmune process related to the bile ducts.

A positive result is especially important during a comprehensive examination for PBC. At the same time the mere fact of detecting AMA-M2 does not automatically mean the presence of active disease. The doctor takes the result into account along with liver biochemical parameters, symptoms, medical history, and results of other tests.

When is the test prescribed?

Research may be recommended:

with increased alkaline phosphatase;

with laboratory signs of cholestasis;

for suspected primary biliary cholangitis;

with unexplained changes in liver tests;

for a comprehensive examination in case of suspected autoimmune liver disease;

to clarify the causes of long-term changes in liver biochemical parameters.

What tests can complement AMA-M2?

For a more complete assessment of the condition of the liver, the doctor may prescribe additional tests, including: ALT, AST, alkaline phosphatase, GGT, bilirubin, as well as other autoantibodies and instrumental examinations depending on the clinical situation.

When confirming characteristic laboratory changes and the presence of AMA, the diagnosis of PBC can in many cases be made without the need for a liver biopsy. AASLD

What a positive result could mean

Positive AMA-M2 is an important sign that supports the suspicion of primary biliary cholangitis. However, the result must be evaluated comprehensively.

If AMA is detected but liver function tests remain normal, this does not necessarily mean that the disease has already developed. In such a situation, the doctor may recommend observation and periodic monitoring of liver function tests. AASLD

Preparation for analysis

For research, it is used venous blood.

Before taking the analysis, it is advisable to:

follow the laboratory's recommendations regarding food intake;

avoid significant physical exertion and alcohol the day before;

Sit quietly for a few minutes immediately before blood collection;

inform the healthcare professional about the medications you are taking.

You should not stop taking prescribed medications on your own. before analysis. If the study is performed simultaneously with other tests, preparation should be coordinated with the laboratory.

Important to know

AMA-M2 is not a test that independently determines liver status or confirms PBC in every person. Its value is greatest in complex diagnostics together with cholestasis indicators and other examination data.

It is advisable to discuss the results of the analysis with a doctor, especially if alkaline phosphatase, GGT, bilirubin are simultaneously elevated or there are symptoms associated with impaired liver and biliary tract function.

Antimitochondrial antibodies (AMA-M2) — a laboratory blood test that helps detect an autoimmune reaction, which is primarily characteristic of primary biliary cholangitis (PBC) — a disease in which the small bile ducts of the liver are gradually affected.
