Description

Complex "Preoperative II"« — is an extended package of laboratory tests necessary for a complete assessment of the patient's health status before a planned surgical operation or invasive diagnostic procedure. The complex is formed in accordance with the standards of preoperative examination to minimize anesthetic and surgical risks.

The test results allow the surgeon and anesthesiologist to assess the blood's ability to clot in advance, detect hidden inflammation or anemia, check the functional capacity of the kidneys and liver, and make sure there are no blood-borne infections.

Composition of the complex and studied indicators

  1. Clinical blood test (CBC + leukocyte formula + ESR):

    • Assessment of hemoglobin and red blood cell levels (diagnosis of anemia).

    • Determination of leukocyte count and leukocyte shift (detection of acute and chronic inflammatory processes).

    • Assessment of platelet levels (the primary link in blood clotting).

  2. Extended coagulogram (blood clotting system):

    • Prothrombin time (PT) / INR (INR): assessment of the extrinsic pathway of coagulation.

    • APTT (activated partial thromboplastin time): assessment of the intrinsic coagulation pathway.

    • Fibrinogen: the main protein involved in blood clot formation.

  3. Biochemical indicators:

    • ALT and AST: assessment of the condition of the liver and heart muscle.

    • Total bilirubin: assessment of pigment metabolism and suspicion of biliary tract pathology.

    • Creatinine and Urea: The excretory function of the kidneys is important for the excretion of anesthetic drugs.

    • Glucose: control of carbohydrate metabolism and exclusion of latent diabetes mellitus.

  4. Infection screening:

    • HBsAg (hepatitis B virus)

    • Anti-HCV (hepatitis C virus)

    • Syphilis (Treponema pallidum, total antibodies)

    • HIV 1/2 (HIV infection, antibodies and antigen)

  5. Complete urinalysis (CUR):

    • Assessment of the condition of the urinary system and detection of hidden urinary tract infections.

Indications for appointment

  • Elective surgical interventions of any degree of complexity (general surgery, traumatology, gynecology, urology, plastic surgery, etc.).

  • Invasive diagnostic procedures requiring anesthesia support.

  • Mandatory examination before hospitalization in a surgical hospital.

Preparation for the study

  • Blood donation: It is taken strictly on an empty stomach in the morning (after 8–12 hours of fasting). The day before the study, fatty foods, alcohol, and excessive physical exertion should be excluded. It is allowed to drink clean, non-carbonated water.

  • Urine collection: On the eve of the study, collect the first morning portion of urine in a special sterile container after thorough hygienic toilet of the genitals.

Frequently Asked Questions (FAQ)

1. How long are the results of this preoperative complex valid?

Preoperative test results are usually valid for 10–14 days prior to the date of surgery. The exact timeframe depends on the requirements of the particular hospital or surgeon.

2. Why is it important to have an extended coagulogram before surgery?

Disorders in the blood clotting system can cause complications: both increased bleeding during surgery and the risk of blood clots in the postoperative period.

Recommendations after receiving the results

  • With the test results ready, contact your treating surgeon or anesthesiologist for a conclusion and permission for planned surgery.
