Offer for the provision of medical laboratory services “Medilab Plus”

1. General provisions

This offer is an official proposal of the medical laboratory "Medilab Plus" (hereinafter referred to as the Contractor) for the conclusion of a contract for the provision of medical services to individuals and legal entities (hereinafter referred to as the Customer).

2. Subject of the contract

The Contractor undertakes to provide the Customer with medical analysis services in accordance with the current price list, and the Customer undertakes to pay for the services provided in accordance with the procedure and on the terms and conditions provided for in this offer.

3. Procedure for providing services

3.1. The customer contacts the medical laboratory “Medilab Plus” for the purpose of conducting tests.

3.2. The Contractor conducts analyses in accordance with quality and hygiene standards, using modern equipment and materials.

3.3. The results of the analyses are provided to the Customer within the established deadlines determined by the Contractor, in electronic or paper form.

4. Cost of services and payment procedure

4.1. The cost of services is determined in accordance with the current price list of the Contractor, posted on the official website of the medical laboratory "Medilab Plus".

4.2. Payment for services is made in cashless form through payment systems specified by the Contractor, or in cash at the cash desk of the medical laboratory.

5. Rights and obligations of the parties

5.1. The Contractor undertakes to:

  • Provide quality and timely medical services.
  • Ensure the confidentiality of information received from the Customer.
  • Comply with all norms and rules stipulated by the legislation of Ukraine in the field of healthcare.

5.2. The Customer undertakes:

  • Provide reliable information necessary for conducting analyses.
  • Pay for services provided on time.
  • Follow the instructions of the Contractor's medical staff.

6. Responsibility of the parties

6.1. The Contractor is responsible for the quality and timeliness of the provision of services.

6.2. The Customer is responsible for the accuracy of the information provided and timely payment for services.

7. Dispute Resolution

7.1. All disputes arising from the performance of this agreement shall be resolved through negotiations between the parties.

7.2. In the event of failure to reach an agreement, the dispute shall be resolved in accordance with the procedure established by law in the courts of Ukraine.

8. Offer validity period

8.1. This offer comes into force from the moment of its publication on the official website of the medical laboratory "Medilab Plus" and is valid until its withdrawal by the Contractor.

8.2. The Contractor reserves the right to make changes to the terms of this offer, with mandatory notification of this on the official website.
