Examination of the testicular membranes (PG050)

Histological material is taken only in medical institutions.
Pathomorphological examination of the testicular membranes — is a histological analysis of the tissues surrounding the testicle (in particular, the vaginal membrane). Most often, such material is obtained during operations for hydrocele, cysts, or when specific inflammatory processes are suspected. Code PG050 involves a detailed study of the structure of the membranes to determine the cause of their pathological change.
Research objective: The main task is to clarify the nature of the thickening of the membranes or the accumulation of fluid between them. The study allows you to distinguish ordinary reactive changes from chronic specific infections (for example, tuberculosis) or rare but aggressive tumors, such as mesothelioma of the vaginal membrane of the testicle.
What does the pathologist evaluate:
Condition of the mesothelial layer: assessment of the lining of the membranes for the presence of proliferation or atypical cells.
Severity of fibrosis: analysis of the degree of connective tissue proliferation, which often occurs with prolonged dropsy.
Inflammatory infiltrate: determining the type of inflammation (acute, chronic, granulomatous) and searching for pathogenic microorganisms.
Presence of inclusions: detection of calcium salt deposits (calcinosis) or other pathological substances.
Vascular changes: assessment of the condition of capillaries and lymphatic vessels of the membranes, which may indicate impaired lymphatic drainage.
Clinical significance: The conclusion with the code PG050 helps the urologist understand the genesis of the disease. For example, if during surgery for hydrocele signs of specific inflammation are found in the membranes, the patient will be prescribed additional examination and specialized treatment. This study is also critically important for the early detection of rare malignant processes that can clinically mask themselves as a common hernia or dropsy.
When is it prescribed:
after surgical treatment of dropsy of the testicle (Winkelmann, Bergmann, Lord operations);
removal of cysts of the spermatic cord or testicular membranes;
pronounced thickening or compaction of the membranes detected during ultrasound;
suspected mesothelioma or other membrane tumors;
recurrent accumulation of fluid in the scrotum after punctures.
