Biopsy of the testicle and its appendix — is a pathomorphological study of tissue samples from the male gonad and adjacent structure (epididymis). The procedure code PG049 involves the analysis of material from both locations, which allows for a complete picture of the condition of the male reproductive system, especially in cases where the pathological process covers the entire organ complex.

Research objective: The main task is the differential diagnosis of tumor, inflammatory and obstructive processes. Since the appendix is often the first barrier to infections, its simultaneous examination with testicular tissue helps to establish whether the lesion is primary or has spread from the urinary tract.

What does the pathologist evaluate:

  • The structure of the appendix (epididymis): looking for signs of chronic epididymitis, the presence of granulomas or cystic dilatations of the ducts.

  • Testicular tissue: assessment of the state of the parenchyma, detection of atypical cells or specific infectious agents.

  • Patency of the vas deferens: assessment of the presence of sperm in the lumen of the appendage ducts (important in the diagnosis of obstructive azoospermia).

  • Nature of neoplasms: Differentiation of benign tumors (e.g., adenomatoid tumors of the appendix) from malignant germ cell tumors of the testis.

  • Degree of fibrosis: assessment of replacement of functional tissue with connective tissue due to prolonged inflammation.

Clinical significance: The conclusion of the pathologist by code PG049 is critical for patients with suspected associated pathology (orchoepididymitis). This allows the urologist to select specific antibacterial therapy or decide on the need for surgical intervention. This study is also part of the diagnostic search for male infertility, as it allows you to detect a block at the level of the appendix.

When is it prescribed:

  • the presence of bulky formations affecting both the testicle and its appendix;

  • chronic pain in the scrotum with thickening of the appendix of unknown origin;

  • suspicion of specific inflammation (tuberculosis, sarcoidosis) with scrotal involvement;

  • diagnosis of the causes of male infertility (exclusion of obstruction or inflammatory destruction);

  • monitoring the condition of tissues after injuries or testicular torsion.
