CA-125 test: how to understand its meaning and why it is important for women

What is CA-125 and why is it being paid attention to?
CA-125 is a protein molecule (glycoprotein) that is naturally produced by epithelial cells of certain internal organs, such as the endometrium, fallopian tubes, peritoneum, and pleura. In healthy women, it can be present in the blood in small amounts. However, if a pathological process begins in the body, especially associated with ovarian tumors, the level of this protein can increase significantly.
CA-125 analysis used as one of the tumor markers — biochemical indicators that help detect or suspect the presence of a tumor. It is especially valuable in detecting epithelial ovarian cancer, which often does not cause any symptoms in the early stages.
How the analysis works
The analysis determines the amount of CA-125 per unit volume of blood. Normally, this indicator is up to 35 U/ml. If the values are higher, it is a signal for further diagnostics. But it is important to understand: the analysis itself does not diagnose, but only indicates the likelihood of pathological processes.
When can a doctor order a test?
If symptoms are present: chronic pain in the lower abdomen, frequent bloating, feeling of pressure or fullness, rapid fatigue
When cysts or masses are detected in the ovaries on ultrasound
After treatment for ovarian cancer - to monitor the effectiveness of therapy or recurrence
If there have been cases of ovarian or breast cancer in the family (especially if there are BRCA mutations)
In women after 40 years of age as part of an extended preventive examination in case of gynecological complaints

What does elevated CA-125 mean?

A level above 200 U/ml does not mean cancer with a 100% probability, but requires a more in-depth examination - pelvic ultrasound, MRI, additional laboratory tests (e.g. HE4), and in some cases - a biopsy.
Why the result can be elevated without oncology
The body responds to an increase in CA-125 levels not only to a tumor process. It is a marker that reacts sensitively to inflammation of the mucous membranes, hormonal changes, and the formation of tissue fluid in the abdominal or pleural cavity. Its increase is also observed with:
endometriosis
pericarditis
hepatitis
tuberculosis
cirrhosis of the liver
pleurisy
benign uterine neoplasms
Therefore, it is important to consider it along with other symptoms and research results, not separately.
How to prepare for the analysis
To ensure reliable results, you need to follow a few simple recommendations:
Donate blood in the morning on an empty stomach (last meal 8–10 hours ago)
The day before the test, avoid physical exertion, alcohol, and fatty foods.
Do not take the test during menstruation - it is better to do it on the 5th–7th day of the cycle
If you are taking hormonal drugs, tell your doctor.
What is the practical significance of the analysis?
For a woman who has undergone cancer treatment, CA-125 is indicator in dynamics. Its increase may be the first signal of recurrence. For a woman without a diagnosis of cancer, this test can help detect the disease at a stage when there are no clinical manifestations. But its effectiveness increases significantly if CA-125 is combined with other markers (for example, HE4) and instrumental diagnostic methods.
What to remember
CA-125 analysis not intended for all women without exception. It is most useful when there are symptoms or in a risk group. If the result is higher than normal, you should not immediately draw conclusions - you need to undergo a full diagnosis. Only in a complex can you assess the real state of health.
The CA-125 test is not a "scary word," but a way to put your health first. It doesn't answer all your questions, but it does tell you when to stop, get tested, and take care of yourself with due attention.
