Anti-PR3 (c-ANCA) – these are IgG class autoantibodies formed against the enzyme proteinase 3, which is found in neutrophil granules. Their detection is an important diagnostic marker ANCA-associated systemic vasculitis, first of all:
granulomatosis with polyangiitis (Wegener's granulomatosis),
microscopic polyangiitis,
less often - eosinophilic granulomatosis with polyangiitis (Churg-Strauss syndrome).
Anti-PR3 is detected in approximately 80–90% patients with granulomatosis with polyangiitis, therefore, it is one of the most specific laboratory tests for confirming the diagnosis.
When is it appropriate to conduct research:
if systemic vasculitis is suspected with involvement of the upper or lower respiratory tract, kidneys, or blood vessels;
with clinical signs such as:
long-term sinusitis, chronic rhinitis, nosebleeds;
cough, shortness of breath, hemoptysis;
proteinuria, hematuria, signs of nephritis;
to monitor the course of the disease and the effectiveness of treatment;
for differential diagnosis between different types of systemic vasculitis.
Interpretation of results:
Positive result: high probability of ANCA-associated vasculitis, most often granulomatosis with polyangiitis.
Negative result: does not rule out the disease, especially in the early stages or with an atypical course, but reduces its likelihood.
In combination with the clinical picture and other laboratory tests, it allows for a more accurate diagnosis.
Preparation for analysis:
no special training is required;
It is recommended to donate venous blood in the morning on an empty stomach (at least 8 hours after the last meal).
