Medi Lab Plus offers PCR testing for Helicobacter pylori with determination of clarithromycin resistance

PCR diagnostics of Helicobacter pylori — Medi Lab Plus

Dear colleagues!

Medi Lab Plus Laboratory offers molecular genetic testing for simultaneous detection Helicobacter pylori and determination of the main genetic markers of its resistance to clarithromycin by real-time polymerase chain reaction (Real-Time PCR).

Why is this research important?

Helicobacter pylori (H. pylori) is a pathogenic gram-negative bacterium that infects the mucous membrane of the stomach and duodenum. The presence Helicobacter pylori in the body often does not manifest itself with pronounced symptoms, so diagnosing the disease can be difficult. Helicobacter pylori is very common: the bacterium H. pylori occurs in approximately 50% people worldwide. At the same time H. pylori-associated diseases (gastritis, gastric or duodenal ulcer, gastric adenocarcinoma, gastric MALT lymphoma) occur in only 15–20% infected patients.

Clarithromycin is one of the key antibiotics used to treat infection Helicobacter pylori. One of the reasons for treatment failure is the resistance of the bacteria to antibiotics, in particular clarithromycin. There are three main mutations in the genome H. pylori, which are known to cause resistance to clarithromycin: A2143G, A2142G and A2142C. These mutations can change the structure of the ribosome affected by clarithromycin and reduce its ability to bind to the ribosome. Detection of these mutations using PCR diagnostics allows the doctor to personalize the treatment approach, select the most effective antibiotics and avoid the use of those that will not bring the desired result.

Material and research method

  • Biomaterial: dung.
  • Method: Real-Time PCR.Molecular genetic diagnostics of Helicobacter pylori by PCR with determination of clarithromycin resistance mutations.

What does the study reveal?

The Real-Time PCR method detects in a stool sample:

  • DNA Helicobacter pylori — by a specific region of the gene ureA.
  • Mutations associated with clarithromycin resistance: A2142G, A2143G, and A2142C in the 23S rRNA gene.

When is it appropriate to prescribe?

Research may be recommended:

  • for diagnosing infection H. pylori in patients with dyspeptic disorders, gastritis, gastric and duodenal ulcers and other gastrointestinal pathologies;
  • patients with confirmed infection H. pylori before prescribing eradication therapy if a clarithromycin regimen is being considered;
  • patients after unsuccessful eradication therapy - to clarify the possible cause of treatment failure and plan further tactics.

Benefits of the study

Features of preparation

In 1 month: stop taking antibacterial drugs and bismuth preparations (De-Nol, Pepto-Bismol).

In 2 weeks: Stop taking medications that affect gastric secretion, namely PPIs (pantoprazole, omeprazole), in consultation with your doctor.

In 24 hours: Stop using laxatives; drugs that affect peristalsis and stool color (castor or vaseline oil, iron preparations); topical preparations - suppositories, ointments, etc.

Immediately before the examination: do not consume alcohol, foods that increase fermentation in the intestines, and foods containing seeds.

For stool testing in breastfed babies, the rules for food and medication intake apply to both the mother and the baby.

Fence features

Feces are collected only in sterile plastic containers with lids specially designed for collecting biomaterial (feces or urine). Feces are not collected for examination during menstruation, in the presence of bleeding hemorrhoids, blood in the urine, or after significant straining during defecation.

The results of the study are an auxiliary tool for clinical diagnosis and should be interpreted by the doctor in the context of the history, clinical picture, and other examination results.
